It cushions the ball of the foot or offloads metatarsal heads depending on whether you put the pad directly on top of the ball of the foot or behind the ball of the foot. If you don't have a Podiatrist and are using the pads for general cushioning for standing long hours or any cushioning you can place them directly ON the painful area. Apply to clean and dry foot.
